Volunteers
Dear Classroom/Chaperone Volunteer:
Thank you for your willingness to volunteer with the Quillayute Valley School District. To ensure the safety of our students and staff, there is a volunteer application process you must complete before you can begin your volunteer assignment. The process includes the following documentation:
Volunteer Information/Release and Hold Harmless Form
This form is required for the protection of the volunteer as well as the district.
This is an additional form to the Release and Hold Harmless Form required for the protection of the volunteer, as well as the district.
Volunteer Expectation Agreement
By signing this form you confirm that you understand the expectations and appropriate behavior while working as a volunteer with district students and staff.
Criminal History Disclosure Statement
Pursuant to the requirements of RCW 43.43.834, RCW 28A.320.150 and RCW 28A.400.303, we must ask you to complete the enclosed Disclosure Statement.
Washington State Patrol Identification and Criminal History Background Check
This form is required by law in order to be in compliance with RCW 43.43.830, RCW 43.43.832, and RCW 9.96A.020. We request this information to obtain Washington State Patrol records of any criminal convictions for felony crimes as well as any offenses against persons, civil adjudication’s of child abuse, and disciplinary board final decisions from the WSP criminal identification system. Please complete sections C and D. Fingerprints are not required. Sign and date the form to indicate that the information you provided is accurate.
Washington State Sexual Misconduct Disclosure Release
This form must be completed for each school district you have been previously employed. If you have no prior school district employment, please check that box, sign, and date the form.
Provide a Copy of Driver’s License
Attach a copy of your current driver’s license, or other photo identification that includes your legal name. This helps to verify identity during the background check.
Board Policies Acknowledgement Form
This form is required for the protection of the volunteer as well as the district.

- Board Policy No. 3421 "Child Abuse, Neglect, and Exploitation"
- Board Policy No. 5011 "Sexual Harassment of District Staff Prohibited"
- Board Policy No. 5253 "Maintaining Professional Staff/Student Boundaries"
Provide a copy of COVID Vaccination Status
Per the Governor's Proclamation 21-14.1, mandating that all school district employees, volunteers, and contractors become fully vaccinated against COVID-19.
In Quillayute Valley School District, volunteers may meet the requirement in one of three ways:
- Provide verification that you are fully vaccinated against COVID-19 (fully vaccinated is considered two weeks following the receipt of the second dose of Modena or Pfizer or two weeks following receipt of Janssen/Johnson & Johnson); or
- Provide proof of an employer approved Religious Exemption/Accommodation; or
- Provide proof of an employer approved Medical Exemption/Accommodation or initiate the process with your medical provider.
Volunteer Application Packets are also available for pick up at the Quillayute Valley School District Office, 411 S. Spartan Ave., Forks, WA or print and complete the forms provided within the links (areas is in bold) below.
