    Welcome!  Thank you for your interest in becoming a school volunteer.  We appreciate your willingness to lend your time, talent, and energy to work with Quillayute Valley School District students!

    The Quillayute Valley School District welcomes volunteers to serve in a variety of roles: general classroom assistants supporting teachers in multiple tasks, participating as a chaperone for events or field trips, contributing to athletic activities, and/or becoming involved in clubs or committees. 

    To ensure the safety of all students and staff, you must complete the Volunteer Application Process prior to volunteering in a school building or activity. Volunteer Application Packets are availble for pick up at the Quillayute Valley School District Office, 411 S. Spartan Ave., Forks, WA or print and complete the forms provided within the links below. 

    Volunteer Information/Release and Hold Harmless Form

    This form is required for the protection of the volunteer as well as the district.

    Volunteer Assumption of Risk

    This is an additional form to the Release and Hold Harmless Form required for the protection of the volunteer, as well as the district.

    Volunteer Expectation Agreement

    By signing this form you confirm that you understand the expectations and appropriate behavior while working as a volunteer with district students and staff.

    Criminal History Disclosure Statement

    Pursuant to the requirements of RCW 43.43.834, RCW 28A.320.150 and RCW 28A.400.303, we must ask you to complete the enclosed Disclosure Statement.

    Washington State Patrol Identification and Criminal History Background Check

    This form is required by law in order to be in compliance with RCW 43.43.830, RCW 43.43.832, and RCW 9.96A.020.  We request this information to obtain Washington State Patrol records of any criminal convictions for felony crimes as well as any offenses against persons, civil adjudication’s of child abuse, and disciplinary board final decisions from the WSP criminal identification system.  Please complete sections C and D.  Fingerprints are not required. Sign and date the form to indicate that the information you provided is accurate. 

    Washington State Sexual Misconduct Disclosure Release

    This form must be completed for each school district you have been previously employed.  If you have no prior school district employment, please check that box, sign, and date the form.

    Provide a Copy of Driver’s License

    Attach a copy of your current driver’s license, or other photo identification that includes your legal name.  This helps to verify identity during the background check.

    Board Policies Acknowledgement Form

    This form is required for the protection of the volunteer as well as the district.

     

    VOLUNTARIO PAQUETE EN ESPANOL

     

    ATHLETIC VOLUNTEERS ONLY will complete and provide the following prior to participating as a volunteer in ANY district sponsored athletic/activity:

    • Receive approval by Head Coach/Advisor to proceed with meeting athletic/activities volunteer requirements.
    • Upon completion of the below requirements, volunteer must be approved by the Quillayute Valley School District Board of Directors

    OSPI Character and Fitness Supplement Form

    If you choose to volunteer for any athletic program you are required to complete this form per the Washington Interscholastic Activities Association (WIAA) Handbook.

    Valid “Hands-On” First Aid Certificate 

    If you choose to volunteer for any athletic program you are required to obtain this certificate per the Washington Interscholastic Activities Association (WIAA) Handbook.

    Valid “Hands-On” CPR Certificate 

    If you choose to volunteer for any athletic program you are required to obtain this certificate per the Washington Interscholastic Activities Association (WIAA) Handbook.

    Completion of SafeSchools Trainings

    If you choose to volunteer for any athletic program you are required to complete all SafeSchools trainings assigned by the district prior to being approved to volunteer in any athletic program. The district Human Resource Office will set up your account at the time of volunteer approval.

    Completion of Washington Interscholastic Activitites Association (WIAA) Trainings

    If you choose to volunteer for any athletic program you are required to complete all WIAA trainings assigned by the district prior to being approved to volunteer in any athletic program. The district Athletic Director will set up your account at the time of volunteer approval.

    Volunteers are an integral part of the district.  We are fortunate to have such a large parent and community group giving their time in various ways!
