Just a reminder for Parents/Guardians that whenever your student receives an Immunization, you need to inform the school's Health Services Department. Please have the provider who administered the shot print you off an updated CIS (not summary view), sign it, and turn it in to the school. It is also a good practice to keep a copy for your own files at home.

Just a friendly reminder that if your student has: Asthma, Food allergies, Hypoglycemia, Diabetes, or any medical concern that does or does not require medication you need to contact us to set up a Care Plan. Care Plans are only good for one school year, so even if you have had one in place in the past, you must bring in documentation for a plan for current school year. NOTE: If your student has a "Life Threatening" issue they will not be allowed to attend school until a Care Plan is established for the current school year.
